It's been stated before that "anyone doing X is automatically suspicious" in a lot of cases is sort of meta and frowned on as a RP thread.

If a chandler starts dabbling in drugs and poisons because they have been poking at their garden some more and got some herbs... the Queen is still not going bless that.

I doubt the Brotherhood would be happy about people moving in on their turf either and that might settle some things out too.

Maybe ask if RPing with Guilded Physicians and/or rooms in the Madison can give some kind of RP bonus to encourage RP with Physicians?

Sure, but what's the proposed solution, because in a non-consensual game giving people the out to never be arrested without permission will end up meaning no one gets arrested. It might need a balance, but there still needs to be some aspect of risk.
